Fuze Health Inc. is seeking a Registered Nurse to support the telehealth care team by remotely outreaching Medicare members and ensuring adherence to a new Stars measure while confirming discharge from emergency departments.

The role emphasizes compassionate, high-quality virtual care with duties spanning patient education, documentation, and collaboration with care teams. The position is a remote, full-time role with a Monday–Friday schedule and EST hours.

Qualifications

• BSN degree required.
• Active, non-restricted compact-state RN license in listed states.
• Minimum 2 years of RN experience.
• 1–2 years of remote telehealth experience.
• Strong computer skills and familiarity with EMR systems.
• Excellent verbal and written communication skills.
• Ability to follow SOPs in a fast-paced environment.
• Strong customer service and interpersonal skills.
• Ability to work independently and in a collaborative care team.

Responsibilities

• Provide remote telehealth patient care and support.
• Conduct virtual patient assessments, reviewing histories and symptoms.
• Accurately document patient conditions and care interactions.
• Educate patients and families on conditions, medications, and self-care.
• Collaborate with other providers to develop individualized care plans.
• Maintain records and ensure regulatory/privacy compliance.
• Pursue continuous education on telehealth protocols and practices.
• Demonstrate empathy and professional communication in all interactions.
