Fuze Health Inc. is seeking a Registered Nurse to support our telehealth care team, performing remote outreach to Medicare members and ensuring compliance with a Stars measure while confirming ED discharge success.

The role involves virtual assessments, documentation, and patient education in a fast-paced, startup environment. The position is remote in the United States with a schedule Monday to Friday and occasional Saturday coverage.

Qualifications

• Bachelor of Science in Nursing (BSN) required.
• Active, non-restricted compact state nursing license with primary state of licensure in one of the listed states.
• 2+ years of registered nurse experience.
• 1-2 years in a remote healthcare or telehealth setting.
• Strong computer skills and familiarity with EMR systems.
• Excellent verbal and written communication.
• Ability to follow SOPs and adapt in a fast-paced environment.
• Strong customer service and interpersonal skills, especially in high-volume scenarios.
• Ability to work independently and in a collaborative team.
• Startup environment experience preferred.

Responsibilities

• Provide remote patient care and support through telehealth platforms.
• Conduct virtual patient assessments, reviewing medical histories and current symptoms.
• Accurately assess and document patient conditions, histories, and care interactions.
• Educate patients and families on medical conditions, medication adherence, and self-care practices.
• Collaborate with other healthcare providers to develop and implement care plans.
• Maintain records and ensure adherence to healthcare regulations and privacy laws.
• Stay up to date with medical advancements and telehealth protocols.
• Demonstrate empathy and professional communication in all interactions.
