The role focuses on post-discharge follow-up, with a schedule that includes a weekday shift and occasional Saturday coverage. Ideal candidates will hold a BSN, an active RN license, and 2+ years of experience, including remote telehealth.

Qualifications

• BSN required and unrestricted RN license in the US.
• 2+ years of RN experience.
• 1-2 years in remote healthcare/telehealth.
• Strong computer skills and EMR familiarity.
• Clear verbal and written communication skills.
• Ability to follow SOPs in fast-paced settings.
• Excellent customer service and interpersonal skills.
• Ability to work independently and in a team.
• Startup experience preferred.

Responsibilities

• Provide remote telehealth patient care to Medicare members.
• Conduct virtual patient assessments and review medical histories.
• Document patient conditions and care interactions accurately.
• Educate patients and families on conditions, meds, and self-care.
• Collaborate with providers to develop individualized care plans.
• Maintain regulatory compliance and privacy protections.
• Engage in continuing education for telehealth practices.
• Demonstrate compassionate communication in all interactions.
