Job summary

Pharmacy Careers is hiring a Remote Patient Care Pharmacist to provide telephonic or video-based medication counseling, adherence support, and clinical monitoring from home. This role focuses on high-quality patient interaction and flexible scheduling for telehealth delivery.

You will engage in MTM, population health support, and collaboration with care teams to optimize therapies and outcomes, all while maintaining documentation standards and ensuring patient safety.

Qualifications

• PharmD or Bachelor of Pharmacy required.
• Active U.S. pharmacist license; multiple state licenses preferred but not required.
• Ambulatory care, MTM or clinical experience preferred.
• Strong communication and virtual rapport-building skills.

Responsibilities

• Deliver telephonic or video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence and side effects; recommend optimizations as needed.
• Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document interventions and communications according to organizational standards.
• Support population-health and disease-management programs.
