Job summary

Pharmacy Careers is seeking a Remote Patient Care Pharmacist to provide telephonic or video-based medication counseling and adherence support from home. This role offers 100% work-from-home flexibility and the chance to improve patient outcomes through MTM and telehealth services.

You will collaborate with physicians and care teams, perform medication reviews, and document interventions to support population-health programs. Prior ambulatory care or clinical pharmacy experience is preferred.

Qualifications

PharmD or Bachelor of Pharmacy degree.
Active U.S. pharmacist license (multiple state licenses preferred but not required).
Experience in ambulatory care, MTM, or clinical settings.
Strong communication and empathy; ability to engage patients virtually.

Responsibilities

Deliver telephonic or video-based medication counseling and education to patients and caregivers.
Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
Document all interventions and communications according to organizational standards.
Support population-health and disease-management programs.
