Job summary

Pharmacy Careers is seeking a Remote Patient Care Pharmacist to provide direct medication counseling, adherence support, and clinical monitoring via telehealth from home. You’ll engage patients and caregivers through telephonic or video visits to optimize therapies.

You’ll collaborate with physicians and care teams, perform MTM and reconciliations, and document interventions per standards while enjoying a fully remote schedule and growth opportunities in virtual pharmacy care.

Qualifications

• PharmD or Bachelor of Pharmacy degree.
• Active U.S. pharmacist license (multi-state licenses preferred).
• Ambulatory care, MTM, or clinical experience preferred.
• Strong communication and empathy for virtual care.

Responsibilities

• Deliver telephonic or video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
• Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document all interventions and communications according to organizational standards.
• Support population-health and disease-management programs.
