Job summary

UnitedHealth Group's Optum Home & Community Care unit seeks a Part Time Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare members. Qualifications

• Active ANCC or AANP certification for NPs or NCCPA certification and state licensure for PAs.
• Unrestricted licensure in practicing state with no disciplinary actions.
• Driver's license and reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other members.
• Document diagnoses and care plans, and coordinate with PCPs.
• Educate patients on diseases, medications, and adherence; address social needs.
• Identify emergencies and intervene appropriately.
