Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage members. You will document diagnoses, educate patients, and coordinate with PCPs to close care gaps.

This part-time role offers flexible scheduling and no on-call requirements. The position requires active NP/PA licensure in the practicing state with appropriate certification and the ability to travel to patients’ homes in Tulare, CA.

Qualifications

• Active NP or PA license in the practicing state with current certification.
• ANCC or AANP Family/Adult/Geriatric certification required.

Responsabilités

• Perform in-home health assessments for Medicare Advantage and plan members.
• Identify diagnoses for care management and document findings.
• Educate patients on disease processes, medications, and adherence.
• Collaborate with PCPs to close gaps in care and coordinate referrals.
