Job summary

Centerstone is seeking a Team Lead for Contract Care Coordination in a fully remote role supporting Military Services. The position oversees a Clinical Care Coordination team, ensuring contract compliance, accurate case tracking, and timely reporting.

The role requires military affiliation and a Master's degree (or Bachelor's with leadership experience). You will guide staff development, oversee outreach to providers, and support operational processes across referrals and documentation while

Qualifications

• Master's degree in Social Work, Psychology or related field required.
• Bachelor's degree with at least 3 years of leadership experience may be considered.
• Minimum of two years' experience providing behavioral health or social services within the military community required.
• Military affiliation required: current or former service member, spouse or family member of an active-duty service member or veteran, or a Gold Star family member.

Responsibilities

• Directly supervises the day-to-day work of Clinical Care Coordination team; conducts regular supervision and team meetings to support operations.
• Monitor care coordination performance metrics and complete monthly audits; submit internal reports.
• Oversee referral triage, case conferencing, documentation review, training compliance, and productivity management.
• Assist in hiring, onboarding and training of new staff; manage caseload coverage as needed.
• Coordinate external provider outreach and serve as a point of contact for service inquiries.
