Job summary

Sailor Health, Inc. is hiring a full-time Care Coordination Case Manager for a US-based remote role. You will own continuity of care for higher-acuity patients, coordinating with psychiatry, IOP/PHP, inpatient, memory care, and social services.

Requirements include 2+ years in case management or care coordination, comfort with telehealth, and a bachelor’s degree in a related field; healthcare experience with older adults is highly desirable.

Qualifications

• Bachelor's degree in social work, nursing, psychology, public health, or related field.
• 2+ years of experience in case management or care coordination in healthcare.
• Working knowledge of behavioral health levels of care and fit determination.
• Experience with older adults, Medicare or Medicare Advantage preferred.
• Clinical licensure or certification is a plus, not required.
• Comfortable with telehealth platforms and learning new tools quickly.
• Exceptional written and verbal communication and relationship-building skills.
• Must reside in the United States and be authorized to work in the US.

Responsibilities

• Identify high-acuity or out-of-scope patients with therapists and care teams.
• Assess needs, insurance coverage, and barriers to care by phone or video.
• Plan an individualized transition-of-care matching to appropriate services.
• Connect by placing referrals, scheduling first appointments, and transmitting records.
