Job summary

Aware Inc. in Miles City, MT seeks a Targeted Case Manager to deliver high-quality case management to adults, children and families within the community.

This full-time role includes coordinating access to medical, social, educational, vocational and recreational services. The position requires a BS/BA in a social work field and at least one year of relevant experience; strong interpersonal skills, ability to work independently in a virtual office, and a commitment to equitable care are

Qualifications

• BS/BA degree in a social work field and minimum of one year of direct work experience in the developmental disability field.
• Ability to develop and implement individual treatment plans; knowledge of behavioral, clinical, and mental health interventions.
• Ability to work independently in a virtual office using a laptop and cell phone.
• Strong interpersonal communication skills; able to work independently and as part of a team, report observations, follow instructions.

Responsibilities

• Provides case management services to individuals with developmental disabilities.
• Assist these persons gain access to Medical, Social, Educational, Vocational, and Recreational Services.
• Caseload and/or billing requirements on a monthly, quarterly, and yearly basis.
• Collaborate with team to ensure quality care and service coordination.
• Document observations and reports accurately; facilitate meetings.
