Job summary

AWARE in Polson, MT is seeking a Targeted Case Manager to join our caring team. Starting pay is $22.00 to $27.33 per hour for a full-time position.

You will provide case management for developmentally disabled individuals, help access medical, social, educational, vocational, and recreational services, maintain monthly/quarterly/yearly caseload and billing, and collaborate with a supportive team.

Qualifications

• BS/BA in social work or related field, and at least 1 year direct work experience in developmental disability.
• Ability to develop and implement individual treatment plans with behavioral and mental health interventions.
• Strong interpersonal and report-writing skills.

Responsibilities

• Provides case management services to individuals who meet the definition of developmental disability.
• Assist these persons gain access to medical, social, educational, vocational, and recreational services.
• Manage caseload and billing on a monthly, quarterly, and yearly basis.

Talents, skills, and abilities:

• A BS/BA Degree in a social work related field; and minimum of one (1) year of direct work experience in the developmental disability field.
• Ability to develop and implement individual treatment plans; with the knowledge of behavioral, clinical, and mental health interventions, and skill building techniques.
• Understanding of the emotional and behavioral needs of assigned clients.
• Interpersonal communications skills necessary to; work independently and as a team member, make observations and to report them accurately, and follow oral and written instructions.
• The ability to facilitate meetings.
• The ability to work independently via a virtual office; using a lap top computer, and cell phone.
