Job summary

Base pay is $21.00 per hour. Responsibilities include case management, PCSP preparation, documentation, and collaboration with interdisciplinary teams and contracted providers to ensure service delivery complies with Medicaid regulations and Abilities First policy.

Qualifications

• Experience in case management with individuals with developmental disabilities.
• Ability to prepare PCSP and related reports.
• Knowledge of Medicaid regulations and DMH guidelines.

Responsibilities

• Manage a caseload of individuals with developmental disabilities and maintain PCSPs.
• Interview individuals and families; collect data and complete required documents.
• Enter casenote documentation into SETWorks within 72 business hours of activity.
• Coordinate with providers to monitor services and authorizations; ensure compliance.
