Job summary

Loving Angels Network is seeking a dedicated Support Coordinator to manage coordination services for participants. You will collaboratively develop and monitor the Individualized Service Plan (ISP) with the participant, family, and team, while linking to needed services and ensuring budget adherence.

The role requires coordinating planning teams, maintaining detailed records, and ensuring safety and rights are upheld. Flexible work-from-home options are available.

Responsibilities

• Coordinate community resources to ensure waiver services are used when appropriate.
• Develop and monitor the Individualized Service Plan with the participant and family.
• Link individuals to needed services and support providers.
• Monitor service delivery and ensure it stays within budget.
• Schedule and facilitate planning team meetings.
• Maintain confidential case records and required assessments.
• Ensure safety, rights, and reporting of abuse or neglect.
• Communicate with the planning team about ISP changes.
