Job summary

Loving Angels Network is seeking a Support Coordinator to manage Support Coordination services for participants, developing and maintaining individualized plans with the person and their family. The role includes monitoring service delivery and coordinating across agencies to ensure budget compliance and quality outcomes.

The position emphasizes person-centered planning, collaboration with the planning team, and maintaining accurate records while supporting participants’ rights and choices.

Qualifications

• Experience working with waiver services or health services coordination.
• Ability to develop person-centered plans and ISP documentation.
• Strong communication and collaboration across families, providers, and agencies.

Responsibilities

• Develop and monitor the Individual Service Plan with the participant, family, and planning team.
• Coordinate services between state and local agencies to ensure comprehensive support.
• Maintain confidential case records and monitoring tools (NJ CAT, ISPs, PCPTs).
• Ensure services stay within designated budgets and monitor delivery of supports.
• Schedule and facilitate planning team meetings and ISP reviews; distribute ISP/PCPT to stakeholders.
