Job summary

Cadence is hiring a bilingual Spanish‑speaking Clinical Navigator (LPN/LVN) to deliver virtual chronic care support for Medicare patients enrolled in Cadence programs. You will conduct outreach, coach patients, and coordinate care under clinical supervision.

You must be an active LPN/LVN with at least 5 years of experience, fluent in Spanish, and capable of remote work with stable WiFi. Compensation is hourly and remote in the United States, with strong emphasis on patient education and care

Qualifications

• Active LPN/LVN license in multi‑state practice.
• Minimum 5 years of clinical experience as an LPN/LVN.
• Spanish language fluency is required.
• Remote work readiness with stable high‑speed internet.
• Active CA LVN license is highly preferred.
• Experience with AI‑assisted clinical tools is a plus.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one‑on‑one coaching for chronic conditions.
• Help patients execute personalized care plans with behavior modification education.
• Perform comprehensive health assessments and readiness to change.
• Educate on disease management, adherence, and prevention strategies.
• Monitor progress and health outcomes with remote tools and follow‑ups.
• Document interactions and connect patients with resources and support.
• Collaborate with clinicians and teams using Cadence Platform, Google Workspace, and Slack.
