Job summary

Cadence is the clinical AI company automating the treatment of chronic disease, delivering virtual chronic care and coordinating action for Medicare patients enrolled in Cadence programs.

You will conduct outbound calls, coach patients on behavior changes, help execute personalized care plans, and document outcomes using Cadence Platform, Google Workspace, and Slack, all while maintaining clinical judgment under supervision.

Qualifications

• Active NBHWC health coaching certification required.
• At least 1 year of experience as a health coach.
• Experience providing patient education and support for chronic conditions.
• Remote work readiness with stable high-speed WiFi.
• Fluency in Spanish required.
• Familiarity with AI-assisted clinical tools and able to discuss AI-supported care.

Responsibilities

• Conduct outbound telephonic patient outreach and one-on-one coaching for chronic conditions.
• Educate and coach patients on behavior change, nutrition, physical activity, and self-management.
• Assess health status, readiness to change, and lifestyle factors.
• Educate on disease management, medication adherence, detection of symptoms, and prevention.
• Document interactions and outcomes; identify barriers to care and connect to resources.
• Collaborate with clinicians and support teams in real time using Cadence Platform, Google Workspace, and Slack.
