Job summary

Cadence is seeking a Bilingual/Spanish Speaking Clinical Navigator (Health Coach) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. This full-time remote role is based in the United States with standard hours from 8:00 AM to 5:00 PM PT/Mountain Time, or 9:00 AM to 6:00 PM PT.

You will coach patients with chronic conditions, educate on self-management, document interactions, and collaborate with clinicians using Cadence, Google Workspace, and Slack.

Qualifications

• NBHWC health coaching certification is required.
• At least 1 year of experience as a health coach is required.
• Remote work readiness (stable high-speed WiFi) is required.
• Fluency in Spanish is required.
• Fluency with AI-assisted clinical tools (e.g., Gemini, Open Evidence) and ability to discuss AI support vs. human judgment.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions (e.g., type 2 diabetes, hypertension, cardiovascular disease).
• Educate and coach patients on behavior modification, nutrition, physical activity, and self-management using evidence-based techniques.
• Perform comprehensive assessments of health status, lifestyle, and readiness to change.
• Educate on disease management, medication adherence, and prevention, following Cadence policies under clinical supervision.
• Monitor progress and health outcomes via check-ins and remote tools.
• Document interactions and follow-up plans in the Cadence platform; connect patients to resources as needed.
• Collaborate in real time with clinicians and teams using Cadence Platform, Google Workspace, and Slack.
