AltaMed is seeking a Social Worker I based in Montebello, CA to coordinate services for Managed Care participants. You will work closely with providers and nurses to advocate for clients, document daily notes, and facilitate referrals. Strong bilingual English/Spanish is highly valued.

Qualifications

• Bachelor's or master's degree in social work from an accredited university.
• Minimum of 2 years of experience in case management and social advocacy.
• Experience working with the homeless population is preferred.
• Managed Care Experience preferred.
• Bilingual English/Spanish strongly preferred.

Responsibilities

Job Overview

Responsible for AltaMed Managed Care participants assigned to the team. This position works closely with providers and nurses to coordinate services and advocate for participants. Responsibilities include, but are not limited to: daily notes, conducting family conferences, assisting with community referrals, regular contacts with participants, and incident reporting.

Minimum Requirements

• A bachelor' or master's degree in social work from an accredited university is required.
• Minimum of 2 years of experience in case management and social advocacy required.
• Experience working with the homeless population is preferred.
• Managed Care Experience preferred.
• Bilingual English/Spanish strongly preferred.

Compensation

$26.78 - $32.48 hourly
