Catholic Charities of St. Paul and Minneapolis is seeking a Social Worker I to support older adults and individuals with disabilities in their chosen home.

The role delivers trauma-informed, culturally responsive services and develops service plans with clients, connecting them to Catholic Charities programs and community partners. Ideal candidates are dedicated to client success, capable of documentation and advocacy, and able to navigate complex systems with care.

Qualifications

• Must have MN social work license (LSW/LGSW).
• Bachelor’s or MSW in social work or related field preferred; or bachelor’s with 1 year relevant experience.
• Pass DHS background check.
• Ability to work with diverse populations.
• Valid driver’s license and insurance.

Responsibilities

• Provides initial and annual assessments, education, and case management services, including in-home visits.
• Develops service plans and coordinates with Catholic Charities and community resources.
• Advocates for services and assists with required paperwork.
• Educates community systems to facilitate linkage, referral, crisis management, and follow-up.
• Maintains complete case documentation per funders/regulatory bodies.
• Coordinates with supervisor and team.
• Uses trauma-informed, person-centered techniques to support clients.
• May facilitate group or individual meetings.
• Mentors other individuals in program.
• May transport clients as needed.
• Other duties as assigned.
