Job summary

Catholic Charities of St. Paul and Minneapolis is seeking a self-motivated Case Manager III to join our Aging and Disability Services team. You will work closely with clients to understand needs and plan services that support independence and well-being.

Responsibilities include developing service plans, conducting intake and annual assessments, coordinating with community resources, and advocating for clients in a trauma-informed, culturally sensitive manner.
