Qualifications

• Bachelor’s or master’s degree in social work, psychology, sociology, human services, or related field preferred.
• Or a bachelor’s degree with at least one year of experience in social work/case management in a public/private social services agency.

Responsibilities

• Perform initial and annual assessments, education, and case management including in-home visits.
• Develop and maintain service plans with clients and resources.
• Advocate for clients and assist with required paperwork.
• Coordinate with community systems for linkage, referral, and crisis management.
• Maintain complete case documentation and records as required.
