Job summary

Catholic Charities is seeking a self-motivated Case Manager III in Aging and Disability Services to serve clients in Saint Paul, MN. You will develop service plans, conduct intake and annual assessments, identify community resources, and ensure accurate documentation to support client progress.

You will advocate for clients, track progress, adjust service plans as needed, educate on available services, and mentor other case managers while coordinating with supervisors to deliver person-centered

Qualifications

• Bachelor’s or master’s degree in social work, psychology, sociology, human services or related field preferred.
• Or a bachelor’s degree with at least 1 year of experience as a social worker/case manager.
• Must pass DHS background check and meet social work standards for case management.
• Ability to work with diverse, low-income populations including homeless or mentally ill.
• Valid driver’s license and insurance if transporting clients.

Responsibilities

• Provides initial and annual assessments and case management including in-home visits.
• Develops service plans and coordinates Catholic Charities programs with community resources.
• Advocates for clients and assists with paperwork for services and benefits.
• Coordinates with supervisors and educates community systems to facilitate linkage and crisis management.
• Maintains complete case documentation as required by funders or regulators.
• Mentors other Case Managers within the program.
• May transport clients as program needs require.
• Performs additional duties as assigned.
