Qualifications

Responsibilities

• Conduct outreach activities via phone, email, and video conferencing to connect with community members.
• Identify and assess the needs of individuals and families seeking support.
• Provide information about available community resources and services.
• Facilitate referrals and connections to appropriate support programs.
• Build and maintain positive relationships with community stakeholders and partner organizations.
• Develop and deliver virtual presentations and workshops to raise awareness.
