Qualifications

• Relevant qualification in social work, gerontology or related field.
• Experience in elderly care or case management.
• Strong communication, empathy and active listening.

Responsibilities

• Conduct remote needs assessments for elderly clients and identify support requirements.
• Develop and implement personalized support plans for each senior.
• Facilitate connections to community services, healthcare providers and activities.
• Provide regular check-ins and companionship via phone, video calls, and online platforms.
• Educate seniors and families on available resources and support systems.
• Advocate for rights and needs of elderly clients within community and healthcare systems.
