Job summary

Service Coordination Unlimited, Inc. is hiring a Service Coordinator I to assess and secure services for participants, monitor plans, and document visits in our systems.

You will support participants, ensure compliance, and provide training to participants and staff, traveling across Allegheny and surrounding counties as needed. The role emphasizes person-centered care, strong communication, and reliability.

Qualifications

• Bachelor’s degree in social work, psychology, or related field required.
• Practicum experience is a plus.
• Excellent communication and strong interpersonal skills are essential.

Responsibilities

• Assess, identify, secure, and monitor services for program participants.
• Respond to participant needs via various communication methods.
• Meet compliance standards and timeframes for assessments and plans.
• Document visits and contacts in required systems in a person-centered manner.
• Provide training and support to participants and consumer employees.
• Maintain internal databases to track participant information and encounters.
• Help participants obtain accessibility adaptations and other services.
• Demonstrate company values in all interactions and duties.
