Summary

We are seeking a Service Coordinator to join our team. This pivotal role involves coordinating services for individuals with disabilities and our older adult population, ensuring they receive the support necessary to enhance their independence and quality of life.

The Service Coordinators responsibilities are:

• Development of the person-centered service plan that address a Participants identified areas of need, goals, intervention, and progress of goals.
• Coordinate services for Participants in relation to their identified needs.
• Monitor authorized services through face-to-face in-home visits, phone calls, person centered planning team meetings, and contact with service providers.

The Service Coordinator duties are performed remotely from home and will require intermittent travel to conduct in-person home visits throughout Lancaster and Berks counties in Pennsylvania

Position Requirements

Must have a bachelors degree in social work, psychology or other related fields with practicum experience OR in lieu of a bachelors degree, must have at least three (3) years case management experience in a social service or healthcare-related setting.

Salary: $24.04 per hour
