Summary: We are seeking a Service Coordinator to join our team. This pivotal role involves coordinating services for individuals with disabilities and our older adult population, ensuring they receive the support necessary to enhance their independence and quality of life.
The Service Coordinator duties are performed remotely from home and will require intermittent travel to conduct in-person home visits throughout Dauphin and Cumberland Counties, Pennsylvania.
Must have a bachelor’s degree in social work, psychology or other related fields with practicum experience OR in lieu of a bachelor’s degree, must have at least three (3) years’ case management experience in a social service or healthcare-related setting.
Salary: $50,000 annually ($24.04 per hour)
