Job summary

Lifesteps, Inc. seeks a Service Coordinator to locate, coordinate, and monitor supports for adults with disabilities in home and community settings. The role requires building strong relationships with partners and providing excellent client service.

The position is hybrid with travel throughout Western PA, including Allegheny and surrounding counties. A 3-year background in social services or healthcare is required along with a degree in a related field or RN, and the ability to obtain

Qualifications

• Bachelor’s degree or RN required.
• Three (3) years' experience in social services or health care setting.
• Must be able to travel using personal vehicle with valid license & auto insurance.
• Ability to obtain ACT 33/34 and FBI clearances.

Responsibilities

• Locate, coordinate, and monitor supports for adults with disabilities.
• Serve as initial contact for clients with strong customer-service orientation.
• Build relationships with partners and providers to ensure quality care.
• Travel within assigned territory; hybrid work with home visits and in-person client visits.
