Job summary

The Center for Human Services (CHS) seeks a Service Coordinator to manage a diverse caseload and coordinate supports for individuals with developmental disabilities. You will collaborate with families, staff, and outside agencies to develop action plans and ensure timely services in the community.

Requirements include a Bachelor’s degree or RN license, reliable transportation, and ability to work in a hybrid/hybrid-remote setting within the service area.

Qualifications

• Bachelor’s degree or RN license required.
• Experience with individuals with developmental disabilities preferred.
• Strong communication and documentation skills.

Responsibilities

• Manage caseload to improve independence and inclusion.
• Develop ISP plans with the individual and team.
• Coordinate services across federal, state, and community programs.
• Monitor service quality and maintain timely case notes.
