Job summary

Valley Mountain Regional Center is seeking a Service Coordinator to advocate for individuals with developmental disabilities and coordinate essential services. The role involves case management, planning, and direct work with families and community partners, across multiple office locations in California.

You will schedule reviews, prepare required documentation, and participate in internal/external committees while maintaining strong communication and a commitment to diversity and inclusion.

Qualifications

• Bachelor's degree in Human Services or related field with 1 year of related experience.
• Or Bachelor's degree in a non-human service related field with 3 years related experience providing training or direct services to persons with developmental disabilities.

Responsibilities

• Case management, planning, assessment, and development of individual program plans.
• Coordinate and facilitate reviews and team meetings.
• Prepare service authorizations, court reports, and Medicaid documentation.
• Advocate, educate, and provide crisis intervention for clients and families.
• Collect and report client data for surveys and assessments.
