St. Luke's University Health Network seeks aClinical Triage Specialist (RN) to provide expert nursing triage in Rheumatology/Infectious Disease/Pulmonology. The CTS will triage patient calls, document assessments, and coordinate care within the Access Center.

Formación

• Graduate of an accredited nursing program.
• Active Registered Nurse licensure in PA and NJ or a multi-state compact.
• Minimum 2 years clinical experience in physician office, home health, critical care or ED.

Responsabilidades

• Answers inbound calls, triages using nursing protocols to determine level of care.
• Document symptoms, nursing assessments, and care recommendations in the EHR.
• Coordinate referrals and scheduling with practices and ancillary services.
• Provide patient education and self-care guidance and escalate as needed.
