Job summary

Qualifications

• Requires active RN or LPN licensure in PA/NJ or a compact state.
• Minimum 2 years of recent clinical experience in physician offices, home health, critical care, or emergency.
• Strong communication and compliance orientation are essential.

Responsibilities

• Answer and triage patient calls using clinical protocols.
• Verify demographics and enter data into the electronic health record accurately.
• Escalate clinical issues to POD team and provide clinical advice per protocols.
• Respond to escalated messages and prioritize patient care appropriately.
• Create a positive patient experience and resolve issues within scope.
• Meet productivity, schedule adherence and quality standards.
• Collaborate with Access Center teams to address clinical concerns, refills or scheduling.
• Document symptoms, assessments and advised actions clearly.
