Job summary

Qualifications

• Graduate of an accredited RN program.
• NH RN license or RN license from another Compact State.
• BLS within 6 months of hire.
• Ergonomically appropriate home workspace and high-speed internet.
• Private workspace for confidential patient conversations and HIPAA compliance.
• Strong communication and customer service skills.
• Computer workstation will be provided.

Responsibilities

• Respond promptly to patient calls and assess needs through active listening and open-ended questions.
• Conduct nursing assessments and provide recommendations using established guidelines, protocols, and algorithms.
• Schedule appropriate provider visits when emergency or after-hours care is not required.
• Assess patients across the lifespan, from newborn through age 100+.
• Provide professional, compassionate customer service and telephone communication.
• Document calls, assessments, and actions in the patient medical record.
• Communicate assessment outcomes to healthcare team members to support appropriate follow-up.
• Follow contingency procedures during internet or power outages.
• Maintain patient privacy and confidentiality.
• Perform other duties as assigned.
