Call 4 Health is seeking an experienced Registered Nurse (RN) to join our 24-hour remote triage team. In this role, you will provide telephone triage and clinical support nationwide, document interactions in EHRs, and assist providers with care coordination from a home-based workspace.

The ideal candidate has an active nursing license, 2–4 years of RN experience (ER/urgent care or ICU), and experience with multiple EHR systems.

Qualifications

• RN with an active license (e-NCL / Enhanced Nurse Licensure Compact).
• 2–4 years of full-time RN clinical experience (ER/Urgent Care, Adult, Pediatric, Ambulatory Care, or ICU).
• Telephone triage experience using electronic triage systems.
• Experience with two or more EHR platforms.

Responsabilités

• Respond to inbound and outbound calls for a multi-state nurse triage service.
• Utilize Schmitt-Thompson telephone triage protocols to assess and triage patients.
• Document patient interactions accurately in client EHR systems and/or the Call 4 Health platform.
• Provide clinical assessments via phone or patient portal based on established protocols.
• Support providers with administrative and clinical tasks, including medication refills, provider notification of results, and post-discharge follow-up.
• Facilitate referrals and assist with prior authorization requests requiring clinical input.
• Collaborate with hospitals, nursing homes, funeral homes, and government agencies as needed.
• Participate in Remote Patient Monitoring (RPM) programs: monitor vital signs and educate on chronic disease management.
