Call 4 Health is seeking an experienced Registered Nurse (RN) to join our 24-hour medical call center as a Remote Triage Nurse. You will perform telephone triage, document interactions in EHRs, and provide clinical assessments to patients nationwide from a home office.

Ideal candidates hold an ADN/BSN, have 2–4 years of RN experience (ER/urgent care or ICU), and are licensed in e-NCL. You will use established protocols, collaborate with providers, and support RPM and care coordination efforts

Formación

• Active, unencumbered e-NCL / Enhanced Nurse Licensure Compact license.
• 2–4 years of full-time RN clinical experience in ER/Urgent Care, Adult, Pediatric, Ambulatory Care, or ICU.
• Experience with telephone triage and two or more EHR platforms.

Responsabilidades

• Respond to inbound and outbound calls for a multi-state nurse triage service.
• Utilize Schmitt-Thompson protocols to assess and triage patients.
• Document patient interactions in client EHR systems.
• Provide clinical assessments via phone or patient portal per established protocols.
• Support providers with administrative and clinical tasks, including refills and notifications.
• Collaborate with hospitals, nursing homes, and government agencies as needed.
• Participate in Remote Patient Monitoring (RPM) programs, including vital signs monitoring and patient education.
• Screen and qualify patients for clinical trials and provide education.
• Take on additional responsibilities to support team success and patient outcomes.
