Job summary

Call 4 Health is seeking an experienced Registered Nurse for remote triage across multiple states. You will conduct telephone triage, document in EHRs, and provide clinical assessments from a home-based setting.

Ideal candidates have ER/urgent care experience, a BSN or ADN, and an active compact license. The role offers flexibility, 24/7 coverage, and collaborative support with providers nationwide.

Qualifications

• RN with an Associate's or BSN degree.
• 2–4 years of full-time RN clinical experience (ER/urgent care, adult, pediatric, ambulatory, or ICU).
• Active, unencumbered e-NCL/Enhanced Nurse Licensure Compact license; willingness to obtain additional state licenses.
• Experience with two or more EHR platforms.

Responsibilities

• Respond to inbound/outbound calls for a multi-state nurse triage service.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal per protocols.
• Support providers with administrative and clinical tasks (e.g., medication refills, referral notifications).
• Coordinate post-discharge and post-operative follow-ups; assist with prior authorization requests.
• Collaborate with hospitals, nursing homes, funeral homes, and government agencies as needed.
• Participate in Remote Patient Monitoring (RPM) programs (vital signs, diabetes/hypertension).
• Screen and qualify patients for clinical trials and educate patients.
