Job summary

CVS Health is seeking a Transition of Care Case Manager to support high‑risk, medically complex members enrolled in Medicare and Medicaid programs. The role emphasizes care coordination, discharge planning and collaboration with interdisciplinary teams to reduce readmissions and improve outcomes.

You will lead ICP development, assist with medication management and educate members and families about plans and community resources, while maintaining 40 hours per week and a home‑based work setup in

Qualifications

• Active and unrestricted RN license in Illinois.
• Minimum 3 years of clinical practice experience.
• At least 1 year in care coordination or with high‑risk populations.
• Proficient in Microsoft Office Suite (Word, Excel, Outlook, OneNote, Teams).
• Ability to work from home/remote setup and maintain a private workspace.

Responsibilities

• Complete outreach and post-discharge follow‑ups within compliance timelines.
• Coordinate transitions of care between institutional and community settings.
• Ensure medication reconciliation and patient understanding of regimens.
• Lead and participate in care team meetings to develop ICPs and ensure care continuity.
• Support education for members and caregivers on care plans and resources.
• Maintain knowledge of payer/plan benefits and communicate clearly with providers and members.
