Qualifications

• Active, unrestricted RN license in Illinois.
• 3+ years of clinical practice experience.
• 1+ year in care coordination or high-risk populations.
• Proficient with Word, Excel, Outlook, Teams.

Responsibilities

• Complete outreach calls and post-discharge questionnaires on time.
• Schedule follow-ups and ensure medication reconciliation.
• Identify barriers to care such as transportation or housing.
• Lead Interdisciplinary Care Team meetings and ICPs.
• Ensure compliance with state and federal regulations.
