CVS Health is hiring a Transition of Care Management Registered Nurse to support high-risk, medically complex members as they move across care settings.

You will lead discharge planning, coordinate post-discharge care, educate about medications, and collaborate with the Interdisciplinary Care Team to ensure safe and effective transitions. A strong RN with Illinois licensure and experience in care coordination is preferred.

Formación

• Associate degree in nursing required.
• Bachelor degree in nursing preferred.
• 3+ years of clinical practice.
• 1+ year in care coordination or high-risk populations.
• Active RN license in Illinois or ability to obtain.

About This Role

The Transition of Care Management Registered Nurse at CVS Health plays a critical role in ensuring high-risk, medically complex, and vulnerable members experience safe and effective transitions across care settings. • Pay: $66,575 - $142,576/year
• Location: Remote, IL
• Schedule: Full-time

What You\'ll Do

• Complete market-specific post-discharge assessments to identify member needs.
• Provide comprehensive discharge planning and facilitate transitions of care.
• Ensure members receive and understand their medications.
• Provide clinical assistance for services and supports based on health needs.
• Identify and engage barriers to achieving optimal member health.
• Lead and coordinate the Interdisciplinary Care Team to develop Individualized Care Plans.
• Facilitate overall care coordination to ensure optimal wellness.
• Coordinate post-discharge meal delivery and assist with securing DME.
• Provide education to members and caregivers on care plans and community resources.
• Ensure compliance with state and federal regulations, including NCQA standards.

Requirements

Education:

• Associate’s Degree in Nursing (REQUIRED)
• Bachelor’s Degree in Nursing (PREFERRED)

Experience:

• 3+ years of clinical practice experience
• 1+ year of experience in care coordination or working with high-risk populations

Licenses & Certifications:

• Active and unrestricted Registered Nurse (RN) License in Illinois or willing to obtain
