Job summary

Sagility seeks an experienced Registered Nurse to join as a Senior Care Specialist in a work-from-home capacity. This seasonal role focuses on supporting members with chronic conditions via telephonic care, education, and coordinated services, with opportunities for bonuses and incentives after joining Sagility.

The position emphasizes Whole Person Care, collaboration with care teams, and documentation in electronic records, delivering tailored care plans and education to improve health

Qualifications

• Active, unrestricted RN license.
• Compact RN license permitting practice in applicable states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Ability to work independently and in a remote environment.
• Proficient computer skills including Word, Excel and Outlook.
• Telephonic nursing or remote care management experience a plus.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME and home health services.
• Navigate healthcare benefits and available care options with members.
