Job summary

Sagility is seeking experienced Registered Nurses to join as Senior Care Specialists in a work-from-home capacity. This seasonal role offers a tentative end date in June 2027 with potential bonuses and incentives based on performance and business needs.

As a Senior Care Specialist, you will assess members over the phone, develop individualized care plans, provide education, and coordinate services to improve health outcomes for Medicare, Medicaid, or group health plan members.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise nursing judgment in remote environment.
• Intermediate to advanced computer proficiency (Word, Excel, Outlook).
• Ability to navigate multiple electronic medical records systems.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and risk.
• Establish health coaching relationships with members and provide education and support.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health.
• Help members navigate healthcare benefits, services, and care options.
• Assess factors affecting health and well-being.
• Collaborate with multidisciplinary teams and providers.
• Promote self-management with education and resources.
• Coordinate care among members, caregivers, and providers.
• Document interactions, assessments, care plans, and outcomes accurately.
