Sagility is seeking experienced Registered Nurses (RNs) to join as Senior Care Specialists in a work-from-home capacity. This seasonal opportunity has a tentative end date of June 2027, with bonus eligibility after joining and during the assignment.

You will assess members by phone, develop individualized care plans, provide education, and coordinate services to improve health outcomes using a Whole Person Care approach.

Qualifications

• Active RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.

Responsabilités

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and risks.
• Establish health coaching relationships and provide education related to chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME and home health services.
• Help members navigate benefits, services and care options.
• Assess factors that may affect health and well-being; collaborate with multidisciplinary teams.
• Promote self-management with education and resources for individual conditions.
• Coordinate care among members, caregivers, providers, and care teams.
• Document interactions and outcomes accurately in applicable systems.
