Job summary

Qualifications

• Registered Nurse licensure in the operating state.
• CCM certification or similar preferred.
• Experience in case management and coordination of care.
• Travel required within assigned Georgia locations and overnight travel may be needed.
• Strong interpersonal and organizational skills with clinical assessment capability.

Responsibilities

• Provides in-person and telephonic Medical Case Management to injured workers and families.
• Discusses current treatment plans with physicians and discusses alternative plans.
• Assesses, plans, implements, and evaluates patient progress.
• Evaluates treatment plans for medical necessity and cost effectiveness.
• Attends discharge planning conferences and coordinates post-acute care strategies.
• Frequent local travel (up to ~60%); overnight travel as needed; meet with patients and care teams.
