Job summary

This field-based position requires independent judgment, travel up to 50%, and the ability to develop personalized care plans, monitor progress, and document outcomes in the member’s EMR.

Qualifications

• Active, unrestricted RN license in Louisiana.
• 2+ years clinical experience with diverse populations.
• Knowledge of community health and social services.
• Experience with EMRs and documentation.
• Proficient in Word, Outlook, and Excel.

Responsibilities

• Conduct field-based assessments and develop personalized care plans.
• Coordinate with healthcare providers and community resources.
• Monitor member progress and adjust plans as needed.
• Document incidents and report to Care Management.
• Travel up to 50% within Louisiana region.
