Job summary

Humana, Inc. is seeking a Registered Nurse Field Care Manager 2 (Physical Health) to assess members, guide families to resources, and implement personalized care plans.

The role supports management of physical, environmental, and psychosocial health challenges with a member-focused approach, reporting to the Louisiana Medicaid Team. The position is field-based with remote/home office work when not conducting community visits, travel up to 50% within Louisiana, and a scheduling framework of 8:00

Qualifications

• Active, unrestricted RN license in Louisiana.
• 2+ years clinical experience with diverse populations.
• Knowledge of community health resources and agencies.
• Experience with EMR and narrative documentation.
• Proficient in Word, Outlook, and Excel.

Responsibilities

• Assess and evaluate members’ needs to achieve optimal wellness.
• Guide members toward resources and coordinate with care teams.
• Develop and implement personalized care plans.
• Collaborate with healthcare providers and community services.
