Humana Inc. is seeking a Field Care Manager 2 (Physical Health) RN in Louisiana to assess member needs, build personalized care plans, and coordinate services with providers and community resources. This role blends field visits with remote work from a home office.

You will engage members and families, perform assessments, and drive care plan implementation while reporting to the Care Management Manager within the Louisiana Medicaid Team. Travel up to 50% is required.

Qualifications

• Active RN license in Louisiana.
• 2+ years clinical experience with diverse populations.
• Knowledge of community health and social services.
• Experience using EMR and narrative documentation.
• Proficient with MS Word, Outlook and Excel.
• Ability to work independently with limited supervision.

Responsibilities

• Engage and empower members and families using holistic strategies.
• Conduct telephonic and in-person assessments.
• Monitor member progress with regular assessments.
• Develop and implement personalized care plans.
• Collaborate with healthcare providers and community organizations.
• Coordinate delivery of essential services.
• Facilitate communication across interdisciplinary care team.
• Align daily practices with organizational objectives.
• Document and submit incident reports.
