Eligibility includes active LA RN license, 2+ years diverse population experience, and proficient EMR and Microsoft Office skills; BSN and CCM preferred.

Qualifications

• Active, unrestricted RN license in Louisiana.
• 2+ years clinical experience with diverse populations.
• Knowledge of community health and social service agencies.
• Experience with electronic medical records and narrative documentation.
• Proficient in Word, Outlook, and Excel.

Responsibilities

• Engage and empower members and families with holistic strategies.
• Conduct telephonic and in-person assessments to guide to resources.
• Monitor member progress toward individualized care goals.
• Develop and implement personalized care plans.
• Collaborate with healthcare providers and community services to ensure quality care.
• Coordinate delivery of essential services and address social determinants of health.
• Facilitate communication across the care team for seamless transitions.
• Align daily practices with organizational goals.
• Document and submit incident reports per policies.
