Job summary

Humana Inc. is seeking a RN Field Care Manager 2 (Physical Health) to assess needs, guide members to resources, and coordinate care across the Louisiana Medicaid team. Field-based role with telephonic and in-person assessments and a remote home-office setup when not visiting members.

You will collaborate with providers and community services to create personalized care plans, monitor outcomes, and ensure quality, cost-effective care. Travel up to 50% within Louisiana is required.

Qualifications

• Active, unrestricted RN license in Louisiana.
• 2+ years clinical experience with diverse populations.
• Knowledge of community health and social services resources.
• Experience using EMR and narrative documentation.
• Intermediate- to advanced computer skills; MS Word/Outlook/Excel.

Responsibilities

• Engage and empower members and families to support recovery and wellness programs.
• Conduct telephonic and in-person assessments to guide members to resources.
• Monitor member progress and adjust care plans to meet goals.
• Develop personalized care plans based on clinical judgement and psychosocial factors.
• Collaborate with healthcare providers and community services to ensure quality, cost-effective care.
• Coordinate delivery of essential services and address social determinants of health.
• Facilitate communication across the interdisciplinary care team for seamless transitions.
• Align daily practices with departmental and organizational objectives.
• Document and submit incident reports per policies.
