Job summary

Humana is seeking an experienced RN, Field Care Manager 2 (Physical Health) to assess and address members’ needs within the Louisiana Medicaid Team. You will guide members and families toward resources, coordinate care, and develop personalized plans to achieve optimal wellness.

You will work primarily in the field with remote home-office duties when not visiting members, travel up to 50% within Louisiana, and collaborate with providers and community resources to ensure high-quality,

Qualifications

• Active, unrestricted RN license in Louisiana.
• 2+ years clinical experience with diverse populations.
• Knowledge of community health and social service agencies.
• Experience using EMR and narrative documentation.
• Proficient in Microsoft Word, Outlook, and Excel.
• Ability to work independently with sound judgment.

Responsibilities

• Conduct telephonic and in-person assessments to evaluate member needs and guide resources.
• Develop personalized care plans based on health status and psychosocial factors.
• Coordinate with healthcare providers and community services to ensure quality care.
• Document and report incidents following applicable policies.
• Support member progress toward wellness goals.
